1. Program Overview
The Washakie County Activity Pass Program ("Program") provides financial assistance in the form of vouchers to eligible at-risk youth (18 and under) in Washakie County to help cover fees for youth activities. By applying for or accepting Activity Pass assistance, you agree to these terms of service.
2. Eligibility Requirements
To be eligible for Activity Pass assistance:
- Youth must be 18 years old or younger at the time of application
- Youth must be residents of Washakie County, Wyoming
- Family must demonstrate financial need
- Youth must be considered at-risk or would benefit from positive activity engagement
- Requested activity must align with program goals of positive youth development

All applications must be complete and accurate. Providing false or misleading information may result in disqualification from the program. The Program reserves the right to:

4. Voucher Terms and Conditions
4.1 Voucher Validity
- Activity Pass vouchers are valid for 90 days from the issue date unless otherwise specified
- Vouchers must be used for the specific activity and provider indicated
- Vouchers cannot be exchanged for cash or transferred to other individuals
- Extensions may be granted for specific circumstances at Program discretion
4.2 Voucher Use Restrictions
- Vouchers may only be used toward approved activities that promote positive youth development
- Vouchers cannot be used for individual private lessons unless specifically approved
- Equipment purchases must be directly related to program participation and pre-approved
- One voucher per child per activity session, unless otherwise approved
5. Responsibilities
5.1 Family Responsibilities
- Ensure child attends and participates in approved activities
- Communicate with activity providers regarding voucher use
- Notify the Program of any changes in circumstances or activity plans
- Use vouchers only as intended and within validity period
- Provide feedback about program experience when requested

- Process applications fairly and consistently
- Maintain confidentiality of applicant information
- Provide clear communication about voucher terms and restrictions
- Work with activity providers to ensure smooth voucher processing
- Support families in accessing appropriate activities
6. Privacy and Confidentiality
The Program is committed to protecting the privacy of all applicants and participants:
- Personal and financial information is kept strictly confidential
- Information is only shared with authorized program staff for evaluation purposes
- Aggregate program statistics may be shared for reporting purposes without identifying individuals
- Families may be invited to share success stories but participation is voluntary
7. Limitations and Disclaimers
7.1 Funding Limitations
- Activity Pass assistance is subject to available funding
- The Program does not guarantee approval for all applications
- Funding amounts and availability may change based on program resources
- Multiple applications per family are allowed but not guaranteed approval
7.2 Activity Provider Relationships
- The Program is not responsible for the quality or safety of activities provided by third parties
- Families are encouraged to research and evaluate activity providers independently

- The Program is not liable for injuries or incidents during activities
8. Program Changes and Termination
The Program reserves the right to:
- Modify eligibility requirements, application processes, or terms of service with notice
- Suspend or terminate the program due to funding limitations or other circumstances
- Discontinue voucher issuance if program goals are not being met
- Update policies and procedures to better serve the community

If you disagree with a program decision or have concerns about program operations:
